"I need to register with a GP before I can get any medical care in the UK — so I have to wait."
✅ FACT
You do not need a GP registration for: NHS 111 (24/7, free), A&E (free, immediate), Pharmacy First (no appointment, treats 7 conditions with prescription medicines), walk-in/urgent treatment centres, and sexual health clinics. None of these require GP registration or immigration status checks. Register with a GP at the same time — but don't delay care while waiting for that registration to be processed.[1]

"In the USA without insurance, I can't see a doctor at all — everything is too expensive."
✅ FACT
Federally Qualified Health Centers (FQHCs) are legally required to provide care regardless of ability to pay — some visits cost $0 on a sliding scale. Telehealth services like Wisp, MDLive, and Teladoc offer doctor consultations for $25–$75. GoodRx reduces prescription costs by up to 80% at any US pharmacy. Urgent care clinics cost $80–$150 vs. $1,000–$3,000+ at ER. If you use the ER, you receive a bill — but you can always apply for financial assistance (charity care) which hospitals are legally required to offer.[2]

"I should apply traditional Nigerian remedies first — things like Agbo, pepper soup, or palm kernel oil on injuries — before trying UK/US medicines."
✅ FACT
Some traditional remedies have genuine supportive value (honey for sore throat, ginger for nausea, turmeric for inflammation) and can be used alongside OTC medicines. However, applying palm kernel oil, butter, or raw egg to burns delays cooling (which must happen for 20 minutes under cold running water) and increases infection risk. Agbo preparations in the UK/USA are not standardised — unknown potency, potential drug interactions with any UK medicines you take. Use traditional remedies as supportive complements, not replacements for proven OTC products.

"I should go to A&E for everything I can't manage at home — that's the safest option."
✅ FACT
A&E is designed for life-threatening emergencies. For non-emergencies, A&E in the UK has average wait times of 4–6+ hours. NHS 111 will direct you to the right service much faster — often booking you into an Urgent Treatment Centre or Pharmacy First within the hour. In the USA, the ER for non-emergencies will result in a very large bill. Urgent Care, telehealth, and FQHCs are faster, cheaper, and often more appropriate for minor conditions.[3]

"The medicines here are stronger than in Nigeria — I should take half the dose to be safe."
✅ FACT
Paracetamol, ibuprofen, and most OTC medicines are standardised globally — the 500mg paracetamol tablet in Boots is the same dose as Panadol in Nigeria. Taking half the dose means inadequate pain/fever relief without added safety benefit. The only adjustment to consider is your body weight — for paracetamol, if you weigh under 50kg, consult a pharmacist about dose. Always take the recommended dose on the label.

Call NHS 111 first — this is a free 24/7 service that requires no registration, no GP, and no insurance. They will assess your symptoms and direct you to the right service: Pharmacy First, Urgent Treatment Centre, or A&E if needed. Alternatively, walk into any Boots, LloydsPharmacy, or independent pharmacy and ask for a consultation — the pharmacist can advise you on OTC treatment and, under Pharmacy First (since January 2024), can prescribe antibiotics for 7 specific conditions without a GP. The strongest OTC painkiller in the UK is co-codamol 8/500mg — available from pharmacy counters (not shop shelves) under brand names like Solpadeine Max, Syndol, and Codipar. It contains codeine (8mg) combined with paracetamol (500mg). It is only sold to adults and should not be used for more than 3 days consecutively — codeine can cause dependence. For most moderate pain, however, combining paracetamol 1,000mg + ibuprofen 400mg taken at the same time is more effective than either alone and is an approach endorsed by NHS clinical guidance. For inflammation specifically (sprains, muscle injury, joint pain), topical diclofenac gel (Voltarol) applied directly to the site is often more effective than anything taken orally.
Yes — since January 2024, the NHS Pharmacy First scheme covers uncomplicated UTIs in women aged 16–64. Walk into any participating pharmacy, ask for a private consultation for cystitis/UTI, and the pharmacist can prescribe antibiotics (trimethoprim or nitrofurantoin) for you completely FREE on the NHS without any GP visit. While waiting or if the pharmacy is closed, buy Cymalon sachets or Effercitrate from the pharmacy to reduce the burning sensation. Drink 2–3 litres of water per day. If you develop fever, back pain, nausea, or vomiting — this suggests kidney infection; go to A&E immediately.
Call NHS 111 first — they will advise whether you need stitches and direct you to the nearest Urgent Treatment Centre (UTC). UTCs treat minor injuries including lacerations, fractures, and wounds — for free, regardless of GP registration. Many have X-ray facilities. If the wound is heavily bleeding, contaminated, or you feel unwell — go directly to A&E or call 999. For small cuts that are clean but gaping, Steri-Strip wound closure strips (available at any pharmacy) can hold small wounds together — apply after thoroughly cleaning and drying the wound.
No — antibiotics are prescription-only in both the UK and USA. They cannot be purchased over the counter like in Nigeria. However, since January 2024, the UK Pharmacy First scheme allows NHS pharmacists to prescribe and dispense antibiotics (without a GP visit) for 7 specific conditions: sinusitis, sore throat, earache, infected insect bite, impetigo, shingles, and women's UTIs. For these conditions, simply walk into a participating pharmacy. For other conditions requiring antibiotics, you need to see a GP (UK) or a doctor/urgent care provider (USA). Using telehealth services (UK: Babylon, Push Doctor; USA: Teladoc, MDLive) is often the fastest way to get an antibiotic prescription without physically seeing a doctor.
Night leg cramps in Nigerians arriving in the UK are extremely common and are most often caused by magnesium deficiency. This is triggered by: cold temperatures causing increased urinary magnesium loss, dietary changes (processed Western foods are magnesium-poor vs. Nigerian vegetables and legumes), stress of relocation, and reduced Vitamin D (which affects magnesium metabolism). Start magnesium glycinate 300–400mg before bed — this usually resolves leg cramps within 1–2 weeks. Ensure adequate Vitamin D supplementation. Stay well hydrated. If cramps are very severe, occur during the day, are one-sided, or are associated with leg swelling and redness — seek urgent medical assessment as these can be signs of DVT (blood clot), which is a medical emergency.
