Duavive and Bijuva are both combined menopause hormone therapies that protect the womb lining in a different way from standard HRT.[1][2] Duavive pairs an estrogen with a non-hormone drug called bazedoxifene. Bijuva (sold as Bijuve in the UK) puts estradiol and progesterone into one capsule. Both need a prescription, and both are for women who still have a uterus.[2][3]
I often see patients come in asking about these two names after reading about them online. They want to know if the drugs are "natural", "safer", or just different packaging. Here is what I tell them.
What Are Duavive and Bijuva?
Start with what is actually inside each one, because the brand names tell you very little.
Duavive is a once-daily tablet containing 0.45 mg of conjugated estrogens and 20 mg of bazedoxifene.[3] The conjugated estrogens replace the estrogen your ovaries no longer make. Bazedoxifene is a selective estrogen receptor modulator, or SERM (a drug that acts like estrogen in some tissues and blocks it in others). Here, it stops estrogen from over-stimulating the womb lining.[1]
Bijuva works differently. Each capsule holds estradiol (the main estrogen your body makes) plus 100 mg of micronised progesterone, taken each evening with food.[2] The US sells 0.5 mg and 1 mg estradiol strengths. The UK lists the 1 mg capsule only.[2][4]
How Standard HRT Works and Why the Womb Lining Matters
To see what makes these two different, you need the standard setup first.
Standard HRT for a woman with a uterus combines an estrogen with a progestogen (a progesterone-type hormone). The estrogen eases hot flushes and night sweats. The progestogen protects the womb lining, because estrogen alone can overstimulate it and raise the risk of endometrial cancer.[4] Many women use a skin patch or gel for the estrogen. Skin delivery bypasses the liver, which is one reason doctors often prefer it when clot risk is a concern.[5]
But two products can mean two sets of side effects. From what I see in practice, the progestogen is where many women struggle, with mood changes, bloating, and breast tenderness. Duavive and Bijuva tackle that problem in opposite ways. Tibolone is a third route, covered in our guide to what tibolone is and how it differs from standard HRT. For every option side by side, see HRT and Menopause Treatments: Types, Side Effects and Safety Explained.
How Duavive Differs From Standard HRT
Duavive takes the first route: it contains no progestogen at all.
Bazedoxifene does the lining protection instead.[1] In the SMART trials, endometrial hyperplasia (overgrowth of the womb lining) stayed under 1%, similar to placebo. Moderate or severe hot flushes fell by 74% at 12 weeks, against 51% on placebo.[6] And breast density and breast pain did not rise compared with placebo, which matters if standard HRT makes your breasts sore.[6]
But Duavive still contains an estrogen. Blood clots and gallbladder disease stay on its warning list.[1] The trials lasted two years, so the long-term breast cancer effect is unknown.[6][7] You also cannot add a progestin, another estrogen, or another SERM on top.[1] One practical perk: you can take it at any time of day, with or without food.[7]
Availability is the other catch. UK licensing covers only women who cannot use a progestogen, and the NHS drug dictionary now lists the UK pack as discontinued.[8][9] It still has approved labels in the US (as Duavee), Canada, and Australia.[1][3][7]
How Bijuva Differs From Standard HRT
Bijuva takes the opposite route. It keeps the progesterone and cuts the pill count.
Standard combined HRT often means two products. Bijuva gives you both hormones in one capsule, and the progesterone is body-identical (its structure matches the hormone your ovaries made).[10] The NHS Specialist Pharmacy Service describes it as the first oral continuous combined body-identical HRT in the UK.[10] In the REPLENISH trial, endometrial hyperplasia stayed at or below 0.36% across all doses after one year, and bleeding settled over time.[11]
But there is a catch. "Continuous combined" means a capsule every day with no break, so the UK label limits it to women at least 12 months past their last period.[4] Starting earlier can mean unpredictable bleeding. The ratio is also fixed, so you cannot adjust the estrogen and progesterone separately. And it is oral only. If you want a patch or gel to bypass the liver, Bijuva is not that product.[2][5]
Standard HRT
- Estrogen plus a progestogen
- Oral, patch or gel
- Each hormone can be adjusted
- Skin options bypass the liver[5]

Spotting, Bleeding and Side Effects to Expect
Whichever option fits you, the first few months can bring surprises, and bleeding is the biggest one.
Both products have been studied for it. Duavive did not increase uterine bleeding or breast tenderness in its trials.[12] Bijuva's bleeding pattern improved over time in REPLENISH.[11] Still, I tell every patient the same thing. Expect some irregular spotting early on. But book a review if bleeding continues past the first few months, or returns after it had stopped. Our guide on whether spotting or breakthrough bleeding on HRT is normal explains the difference.
What worries me more than the spotting is what women do about it. I see the same pattern with hormone pills of every kind.

The most common mistake Lagos women make with oral contraceptive pills is not the one the clinical literature focuses on: it is not about the 24-hour window or interactions with antibiotics, though both matter. The mistake I see most repeatedly is stopping the pill mid-cycle because of a side effect: usually breakthrough bleeding or nausea in the first two weeks: without telling anyone and without replacing the method with anything else. They stop because they are scared by the symptom, they do not come back to me or go to a clinic, and they assume the protection continues after stopping. It does not. The number of unintended pregnancies I have seen traced back to this exact pattern is significant. What I now do with every woman I counsel on the pill is spend five minutes specifically on what to expect in the first cycle, why those symptoms happen, and what to do: and not do: if they occur. The information that prevents the most harm is not about pharmacology. It is about what to do when something feels wrong.
The same applies to Duavive and Bijuva. Call your pharmacist or prescriber before you stop either one.
Who Should Not Use Duavive or Bijuva?
Spotting is manageable. Some medical histories rule these medicines out.
Both contain estrogen, so neither suits women with a history of breast cancer.[13] Estrogen also raises the risk of blood clots in the legs and lungs.[13] Duavive has not been studied in women over 75.[3] Tell your pharmacist about every medicine you take, including herbal products.
And check which leaflet you are reading. In February 2026 the FDA approved label changes for the first six HRT products, removing boxed warning statements on heart disease, breast cancer and dementia, with more expected in batches.[14] Ask which version applies to your product. If estrogen is off the table, ask about non-hormonal options such as fezolinetant and who it is for. In Nigeria, buy hormones only from a licensed pharmacy and confirm NAFDAC registration.
Myth vs Fact: Duavive, Bijuva and HRT
Plenty of confusion survives all of that, so here are the three myths I hear most.
Pharmacist Verdict
For most women, I would still start the conversation with a skin patch or gel plus micronised progesterone, because that setup has the longest track record and the lower clot risk.[5] Bijuva is a sensible choice if you are a year past your last period and want one capsule a day. Duavive suits women who cannot tolerate a progestogen, though you may struggle to find it. One warning I will not soften: never stop or switch hormones on your own because of spotting.
